They include the Bayshore Health Care & Rehabilitation Center, a nursing home; The Willows At Holmdel, an assisted living community; the Pleasant Valley Adult Day Center in Holmdel; an Ambulatory Surgery Center in Old Bridge; a SleepCare Center in Holmdel; a SleepCare Center in Freehold; and two Fitness & Wellness Centers, one in Hazlet and one in Old Bridge.
